- What to bring to your DOT physical exam -
A picture ID (driver’s license, passport, etc.).
List of your Medications and dosages.
Hearing Aids and/or Glasses if needed.
- Diabetes, please bring a recent (within 6 six months) A1C report.
- If on Coumadin (Warfarin) you will need to bring copies of PT/INR lab work.
Compliance report if using a CPAP Machine.
Any other special documentation you have needed to present in the past to get your DOT Medical Certificate.
For best results remember:
Urinalysis is required for the DOT physical. Drink plenty of water and be ready to provide a urine sample.
Try to refrain from smoking, eating salty foods, drinking sugary drinks or any caffeine before your CDL physical. (This helps with blood pressure).
